Important Information for the 2013-2014 academic year.
Beginning August 2013-2014 school year, all Silver Lake College of the Holy Family full-time undergraduate students are required to have health coverage. This requirement insures that each student is in compliance with the Patient Protection and Affordable Care Act which will take effect January 1, 2014.
How will this affect you?
For 2013-2014 all Silver Lake College of the Holy Family full-time undergraduate students must have health insurance coverage. Students have 3 options and must choose one of the following:
- Option 1 – Obtain coverage under a parent’s insurance plan. If a student is covered under a parent’s Insurance plan, please email a copy of the insurance card to: firstname.lastname@example.org or FAX a copy of the insurance card to: 920-684-7082 Attention Jan Algozine.
- Option 2 – Obtain coverage from an alternative insurance plan. (Check state and federal options for low income families). If the student is covered under a state or other health insurance plan, please email a copy of the insurance card to: email@example.com or FAX a copy of the insurance card to: 920-684-7082 Attention Jan Algozine.
- Option 3 – Purchase the Silver Lake College Student Health Insurance Plan through WAICU/WPS for an annual premium of $1,489.20 for one year of coverage (August 1, 2013 – July 31, 2014). The cost of the insurance will be added to the student’s bill: $620.50 in the Fall 2013 (5 months of coverage) and $868.70 in the Spring/Summer 2014 (7 months of coverage). Once the student is enrolled in the WAICU/WPS Plan by Jan Algozine, insurance cards will be sent directly to the student’s permanent/home address. What about part-time and Graduate students?
Silver Lake College part-time and Graduate students can enroll in the Silver Lake College student WAICU/WPS Health Insurance Plan. Call WPS directly 1-800-221-5573.
